When a person applies at CCM for assistance, CCM records information about the applicant. Since this information may be shared with other organizations and service providers within our community, every applicant must sign a release consent form so CCM is able to coordinate services and resources with, but is not limited to, Cabarrus and Rowan Depts. of Social Services, the Salvation Army, Community Free Clinic, and area churches.
By law when an applicant contacts a church or other organization for help, and that church or organization calls CCM for verification of applicant's situation, a valid signed Release Consent form must be on file or faxed to CCM before CCM can share applicant's information with that church or organization.
Also if a church or organization is sending an applicant to CCM for help, a Release Consent form needs to be signed and faxed to CCM.

The NS_ChurchVolunteerBrochure.doc file is a brochure for churches to personalize and use for Night Shelter volunteers. The layout of the information is designed to be printed on both sides of a sheet of paper and then folded into thirds for distribution.
The idea is for the churches to open the Microsoft Word file ( NS_ChurchVolunteerBrochure.doc ) in their internet browser and use the "save as" function from the "file" menu pulldown to save the DOC type file on their system. Then use their word processor to personalize the saved file.
You can personalize the brochure information by filling in the name of the church and the coordinator's name. After saving the modifications, print the brochure, fold it, and hand it out to potential volunteers.
